چکیده

Abstract Background The aim of this study was to use recent evidence investigate and update volume–outcome relationships after open surgical repair (OSR) endovascular (EVAR) abdominal aortic aneurysm in England. Methods Hospital Episode Statistics (HES) data from April 2006 March 2018 were obtained. primary outcome in-hospital death. Other outcomes included duration hospital stay, readmissions within 30 days, critical care requirements. Case-mix adjustment age, sex, HES year, deprivation index, weekend admission, mode type procedure co-morbidities. Results Annual volume all repairs combined appeared be an appropriate measure volume. After case-mix adjustment, a significant relationship between mortality seen for OSR (P < 0·001) but not EVAR = 0·169 emergency P 0·363 elective). effect extend beyond 60 per year volumes above 100 year. There no stay or 30-day readmissions. In patients receiving OSR, higher associated with longer care. Conclusion Higher annual all-procedure significantly lower such EVAR. enough on other outcomes.

Endovascular abdominal aortic aneurysm repair.

The early 1990s ushered in the era of endovascular aneurysm repair (EVAR). Diffusion of this technology, although widespread, has been met with enthusiasm by some and caution by others. Advocates of traditional open surgical techniques maintain that EVAR is costly and that long-term outcomes for patients are inferior. The objectives of the present review are to use a case scenario to highlight ...

Regarding "endovascular vs open repair for ruptured abdominal aortic aneurysm".

OBJECTIVE Endovascular repair (EVAR) of ruptured abdominal aortic aneurysm (rAAA) has become first-line therapy at our institution and is performed under a standardized protocol. We compare perioperative mortality, midterm survival, and morbidity after EVAR and open surgical repair (OSR). METHODS Records were retrospectively reviewed from May 2000 to September 2010 for repair of infrarenal rA...

Endovascular or open repair for ruptured abdominal aortic aneurysm?

The three year results of the IMPROVE trial, in the related article by the IMPROVE Trial Investigators (doi:10.1136/bmj. j4859), will change clinical practice in favour of endovascular repair for patients with suspected ruptured abdominal aortic aneurysms (AAA). It is important to stress, however, that long term trials of endovascular compared with open surgery have reported diverging results f...

Due to the risk of rupture, surgical repair is offered electively to individuals with aneurysms greater than 5.5 cm in size. Traditionally, conventional open surgical repair (OSR) was considered the first choice approach. However, over the past two decades endovascular aneurysm repair ...
